experience to join our team on a travel or contract basis. This

Social Worker Overview:
This role is responsible for managing the healthcare needs,
facilitation and achievement of quality and cost outcomes of all
patients. The case manager, utilizing a multidisciplinary team
approach, negotiates, procures, coordinates and monitors services
and resources needed by patients. The case manager is responsible
for reviewing the patient at admission, and in some instances prior
to admission, and concurrently throughout the hospital stay for the
appropriate level of care and utilization of resources.
Social Worker Setting/Shift:
Environment: Acute Care/Medical
Start Date: ASAP
Shift: 4x10; Days
Social Worker Duties:
- Provide education and consultation regarding resources,
collaborate to develop a discharge plan and facilitate and monitor
its implementation.
- Assist patient/family to adjust to and cope with illness,
diagnosis, treatment options, and prognosis.
- Through advanced practice skills mobilizes resources to reduce
risk, serves as a liaison between the hospital and the
community.
- Offers crisis interventions to patient/family.
- Strives for superior performance by consistently providing a
product or service to leadership and staff that is recognized as
ultimately contributing to the patient and family experience.
- Recognizes and demonstrates understanding of patient and
family-centered care.
- Ability to maintain confidentiality.
- Proven collaboration and negotiation skills with providers,
patients, families, insurance organizations and government
agencies.
- Proven ability to work independently.
- Demonstrated ability to establish and maintain relationships with
physicians.
- Knowledge of local community resources required.
- Knowledge of Medicare, Medicaid, third party commercial payers
and reimbursement required.
- Skills in data comprehension, research, analysis, interpretation
and judgement necessary to prepare a comprehensive summary report
for files, management, legal and/or regulatory agencies upon
request.
Social Worker Qualifications:
- California State LMSW License - REQUIRED
- A minimum of 1 years of acute care experience - REQUIRED
- Basic Life Support (BLS) Certification - REQUIRED
- 1+ Year in an Acute/Hospital Setting - STRONGLY PREFERRED
